Parent Consent Form

Purpose:   To ensure parents understand the program structure, expectations, communication methods, and permissions.

Program Participation Consent

I give permission for my child to participate in the NRG In Motion Foundation youth leadership program, including all scheduled activities, discussions, and challenges.

Communication Consent

I agree to receive program updates via:

• Text message

• Email

These updates may include reminders, schedule changes, weekly summaries, and important announcements.

Attendance Expectations

I understand:

• Consistent attendance supports my child’s success.

• Missed sessions can be made up online or in the next cohort.

• I will notify staff if my child will be absent.

Technology Use Consent (Hybrid Program)

I give permission for my child to use:

• Tablets

• Online modules

• Digital journals

• Program apps or platforms

NRG will ensure safe, supervised use.

Parent Engagement

I agree to support my child by:

• Asking about weekly challenges

• Encouraging participation

• Reviewing journal entries (optional)

• Attending the final celebration (if available)
